This is a requested time. We confirm it once a clinician approves your treatment and a nurse is assigned. If it isn’t available we’ll call you with options.

Let your clinician know.

Your answers go only to the clinicians and nurse caring for you. A clinician reviews every request; the website never decides whether treatment is right for you.

Clear expectations.

Please read each document. You can ask your clinician any questions before treatment.

Typing your name and submitting is your electronic signature (California Uniform Electronic Transactions Act). We record the date, time and document version.

One last look.

Submitting sends your request to our care team. No appointment is confirmed and no charge is made until a clinician approves your treatment and a nurse is assigned.
